There is no specific format for such declaration but make it in the regional language that "we do hereby agree to to get the establishment covered by EPFO voluntarily and accord permission to the employer to deduct the contributions due from us from our monthly salaries from the date of giving effect to EPF".
The above statement is not required as per law to be submitted to the appropriate authority who can cover an establishment voluntarily. You can check with the local/ sub regional office of the EPFO regarding coverage of your establishment. The approval has to come from the Central Provident Fund Commissioner and not from the sub Regional Office, I understand.
